I work for a major cable company in maintanance. I work in the up-keep and repair of basic cable, digital cable and cable boxes, high speed internet, networking, and telephony.

Most of what I have seen are due to mistakes during installation or customer tampering (moving things around).
On some occasions with chronic problems they can be a combination of in-house and cable system problems.
Eliminating certain components systematically will usually lead to the root of the problem.
